Since 5S is one of the foundations in any company, it is normally a pretty good place to start. I have put together a 5S spreadsheet that you can begin using today. There are 2 sections to the 5S Scoring Criteria and Check Sheet.

The first section is the scoring criteria. This means people will be scoring each area they are inspecting from the same criteria. This is a step towards standardization, which is a good thing. Each area is detailed out with what it would take to score the related score. At the higher levels, it is understood that that level will require all of the other criteria below, in addition to the new criteria.

The second section is the actual scoring sheet. On this sheet, you should have the people performing the 5S inspections to write their score on this sheet. There is also a section to the right of the page where they can put any suggestions towards improvement that they may see. This helps with the employee involvement. Then you just need to ensure that the improvements are done.
